EQC Year-End Wrap up

As we wrap up our final Ensuring Quality Care (EQC) course for this year, I wanted to reflect on EQC in 2021.

We've met some amazing people over the last year. We want to thank our wonderful EQC participants for their commitment to both the learning process and to their important roles as caregivers

This was a busy year with some significant changes. We have offered nine, full EQC classes this year. In April, we completely updated our EQC course offering, per State requirements, which was a significant undertaking. 

EQC numbers, at a glance:

  • We served 405 students total, between our regular EQC classes and urgent options.
  • We had a 91% course completion rate (i.e. percentage who successfully completed the requirements for the course)
  • Of those who successfully finished the course, 97% passed the State exam, and our students had an average exam score of 94%.

Access to Resident Records

Who has access to resident records, including narratives, care plans, MARs, etc.?  The resident, legal guardian, AFH/ACH care staff, Multnomah County ACHP, and/or Oregon Dept. of Human Services (ODHS) staff (APS, licensing, Long-Term Care Ombudsperson, case management) and people the resident has explicitly given authorization to view their personal information have access to a resident's records.

If a family member or friend is requesting to see a resident's progress notes or anything else in the resident's records, you should always ensure that person has written authorization from the resident to do so.
